Applying to Become an OFA Instructor

 

Requirements

  1. Have an appropriate up to date Occupational First Aid Qualification, at merit level or better.
  2. Have a PHECC recognised CFR instructor certificate.
  3. Attendance at an Occupational First Aid Instructor training course is recommended. However such training is not mandatory as in some circumstances candidates may have sufficient competence and experience without the need to attend such a course.
  4. Send an application form with payment to OFAAA.
  5. Send a set of lesson plans for a Fetac Level 5 OFA course to OFAAA.

The Application Form
To attend an assessment to become an Occupational First Aid Instructor you must fill in the application form  and send it to the OFAAA office. The normal way to do this is on-line by pressing ‘submit’ at the end of the form. You can also print off the form and send it by post. Click her to view the application form

 

Fees
The fee for an individual Assessment & Registration is €500 + 23% VAT: Total €615.00

 

Acceptance of Application
When your application and fees have been received by the OFAAA you will receive confirmation by email (or by letter) that you have satisfied the criteria for assessment and will be provided with a time to attend on the scheduled day. 
You will also be asked to send a set of lesson plans that you have prepared to carry out a Fetac Level 5 Occupational First Aid Training Standard course.

 

Protection for Learners
A candidate may ask to defer an assessment up to ten working days before the notified due date and will be assigned another date. It is the policy of the OFAAA that fees will be forfeited if a candidate cancels an application within 10 working days of the notified date or fails to attend for assessment.

 

Attending an Assessment
Two weeks before the assessment you should send a set of lesson plans to the OFAAA. When attending for assessment you must bring

  1. Proof of identity [driving licence or passport – or other card with a photograph, e.g. FAS Safe Pass card.
  2. Proof of your up to date Occupational First Aid Qualification.
  3. Proof of your up to date PHECC recognised CFR Instructor Qualification.
  4. A passport-size photograph – for use on your OFA Instructor’s card.

 

Lesson Plans
A candidate for Assessment as an Occupational First Aid Instructor must develop a set of lesson plans that would be used to carry out a course of training. The lesson plans will cover 24 hours of training using the Fetac Level 5 Occupational First Aid Standard. The lesson plans will be part of the assessment and must be sent to the Agent not later than 2 weeks before the date of the assessment. The preferred format for receipt of the lesson plans is through an email attachment. However, hard copies of the lesson plans can be sent to the OFAAA, Ballycoolin Business Park, Blanchardstown, Dublin 15.
